Overview
Long-Term Disability (LTD) benefits are available through an integrated disability program that supplements the Short-Term Disability (STD) plan. LTD begins after 13 weeks of STD. The LTD benefit provides 60 percent of base pay up to a maximum benefit of $15,000 per month.**
For eligible employees with a base salary of $50,000 or less, this is a company-paid benefit. If you are eligible for this benefit, you will receive imputed income during the year reflecting the value of the company paid premiums, which is considered taxable income. This will allow any benefits paid to you from the plan to be on a tax-free basis.
Eligible employees with a base salary greater than $50,000 will pay for their LTD coverage on an after-tax basis. This will allow any benefits paid from the plan to be on a tax-free basis.

Eligibility
Regular status, salaried employees scheduled to work at least 20 hours per week are eligible to participate in the LTD plan.
Eligible employees with an annual base salary of $50,000 or less are automatically eligible for and enrolled in the LTD plan as of the first day of work.
Employees with an annual base salary greater than $50,000 are automatically enrolled in the LTD plan as of the first day of work and may elect to waive coverage within 31 days.

Cost
For employees with a base salary of $50,000 or less, the company pays the full cost of the LTD plan.
Employees with an annual base salary greater than $50,000 pay the full cost of the LTD plan. The cost per $100 of annual base salary is $0.40. For example, if your base salary is $60,000: $60,000 times $0.40/$100 = $240 per year ($20 per month).
