Spouse Life and AD&D

Overview

The spouse life plan pays benefits to you upon the death of your spouse. You may purchase spouse life insurance coverage in increments of $20,000 to a maximum of $200,000.

Under the spouse life plan, your spouse will also receive spouse AD&D insurance coverage. Spouse AD&D insurance provides additional benefits to you if your spouse is seriously injured or dies in an accident. The amount of spouse AD&D insurance coverage is equal to the spouse life insurance coverage you purchase through this plan.

The insurance carrier for spouse life/AD&D coverage is the Metropolitan Life Insurance Company.

Additional Details

Comprehensive information about this benefit plan is contained within the Life and AD&D Benefit Plan Summary (PDF). In addition, below is some important information about the plan:

  • Medical Evidence of Insurability

If you elect more than $40,000 in coverage when your spouse is first eligible for Spouse Life Insurance, your spouse will be required to provide medical evidence of insurability to MetLife. If your spouse's application for the amount over $40,000 is not approved, coverage will be limited to $40,000.

After the initial eligibility date for spouse life coverage, your spouse will be required to provide medical evidence of insurability if you previously waived coverage or if you are electing to increase coverage during annual benefits enrollment or during a qualified status change.

After you make your election, MetLife will send you an email with a link to its Web site so that you can complete the evidence of insurability form. You may complete the form online with your spouse's information; however, you will need to print the completed form, sign it, have your spouse sign it and mail the original to MetLife. Coverage will become effective the first of the month following the date MetLife approves the evidence of insurability form. If the form is not approved, the amount of insurance you had in effect prior to applying for the increased coverage will continue.

  • Non-smoker Discount

Your spouse may be eligible for a nonsmoker discount under the spouse life/AD&D plan. To be eligible, your spouse must not have used tobacco products during the previous six months and your spouse must not intend to use tobacco products in the future. Tobacco products under the plan are defined as cigarettes, cigars, pipes, chewing tobacco and snuff.
