Overview
The Child Life plan pays benefits to you upon the death of your child. You can purchase Child Life coverage for your eligible children in increments of $5,000 to a maximum of $25,000.
The insurance carrier for Child Life insurance is the Metropolitan Life Insurance Company.

Medical Evidence of Insurability
When your child is first eligible for Child Life Insurance, medical evidence of insurability (EOI) is not required.
After the initial eligibility date for Child Life coverage, your children will be required to provide (EOI) if you previously waived coverage or if you are electing to increase coverage during annual enrollment or during a qualified status change. A separate EOI questionnaire will need to be completed for each applicable child.
After you make your election, MetLife will send you an email with a link to the Met Life Web site. For children under 18 years of age, you may complete and submit the EOI form online. For children age 18 and over, you may also complete the EOI form online; however, you must print the completed form, sign it, have your child sign it and mail the original to MetLife. Coverage will be effective the first of the month following the date MetLife approves all your EOI forms. If an EOI form is not approved, the amount of insurance you had in effect prior to applying for the increased coverage will continue.
Beneficiary
You, the employee, are automatically the beneficiary for Child Life Insurance.
Eligibility
You are eligible to elect Child Life coverage if you are a regular status, salaried employee of Travelers scheduled to work at least 20 hours a week. You may elect coverage for your dependent children (defined as your natural, adopted, or step children or children of your domestic partner) and your sponsored dependents (defined as a child for whom you have legal guardianship). In general, dependent children and sponsored dependents must be unmarried, under age 19, or under age 25 if a full-time student.
In order to enroll your domestic partner's children, you must complete the Domestic Partner Affidavit and Agreement and the Certification of Domestic Partner Tax Status forms (PDF).*
Cost
Premiums for Child Life are based on the amount of coverage you elect. Premiums are deducted from your paycheck on an after-tax basis.
