United Healthcare Choice Plus Plan Overview

Overview

The United Healthcare (UHC) Choice Plus Plan uses the Choice Plus provider network, which is UHC's broadest national network. The plan offers both in- and out-of-network benefits. It is an "open access" plan, which means that you do not have to select a primary care physician. If you need to see a specialist, you can do so without a referral. The UHC Choice Plus Plan is offered to employees who have access to at least two primary care physicians within 15 miles of their home or office location address.

  • If you are a Virtual Office employee, your home address is used to determine your access.

  • If you are a resident of Hawaii, the only available health plan is Health Plan 200. Additional information on Health Plan 200 is included in the Hawaii Medical Plan Summary (PDF).

  • Please review the UHC Choice Plus Plan Summary Plan Description (PDF) for detailed information concerning the plan.

Eligibility

You are eligible to participate if you are a regular status, salaried employee of Travelers scheduled to work at least 20 hours a week. Your dependents are also eligible for coverage if they meet the eligibility requirements and you elect coverage under the plan.

Cost

You and Travelers share the cost of your medical coverage. When you enroll in the plan, the company will inform you of your share of the premium. Your premium is based on the plan that you choose, the coverage level you select, your annual base salary, and the smoking status of you and your spouse or domestic partner as applicable. Condition Management: UHC offers a Condition Management program to UHC participants that includes education and phone-based support to plan participants with chronic medical conditions. For additional information about the Condition Management program, call 866.676.0740.

Tobacco Cessation: Enrolled employees and their adult dependents who are over age 18 are eligible for a tobacco cessation benefit to help them stop using tobacco. The Tobacco Cessation program provides access to a personal coach and Nicotine Replacement Therapies.

Provider Directory: To find a provider, use the UHC provider directory. Select "Physicians & Facilities" and then select "UnitedHealthCare Choice Plus" from the drop down menu.

The UHC Choice Plus Plan uses the Medica Choice network in the greater Minnesota and Western Wisconsin area. Greater Minnesota and Western Wisconsin area participants should use the Medica provider directory to find a doctor. After selecting the "Member Through Work" link, use the "Search by Network" option and under "Network" select "Medica Choice with UnitedHealthcare Choice Plus."

Note: There is no difference in benefits if you use an in-network or out-of network provider for this plan. However, if you do use an in-network provider, you are likely to save out-of-pocket costs because the co-insurance you pay is based on a negotiated fee representing savings of up to 30 percent.
